I also address the emotional and mental aspects of disease, and work with the body as metaphor. Chronic disease can be any number of clearly diagnosed diseases or it can be the messy mishmash of vague, nagging symptoms (ex: fatigue, digestive upset, acne, hair loss, pain, etc..) that make you feel bad, but your MD says all your lab tests are "normal" and then offers you an antidepressant.
